3. CONSENT TO VIDEO OCTOBER 31 REHEARSAL

The CCO' marketing and communications volunteers, Tai and Tetsuya, want to video a CCO rehearsal for use on the CCO website, instagram, and other social media. They have arranged for the filming to take place at the October 31 rehearsal at St. Barnabas Church.

We would like written consent from the musicians who will be performing at that rehearsal, giving the CCO your permission to use your image on the CCO’s various social media platforms.

A Consent Form will be at the entranceway of St. Barnabas Church at the rehearsals for the next three weeks. Please stop and sign one. Thank you.
